I think that the importance of obtaining a patient's medical history is to gather subjective data from the patient and/or the patient's family so that the health care team and the patient can collaboratively create a plan that will promote health, address acute health problems, and minimize chronic health conditions.

  4. This is awesome! thank you for sharing. These 3 strategies are at the core of motivational interviewing and getting the client to change talk. Empathizing, open-ended questions, affirmations, reflections and summarizing. As clinicians, these factors are key as we enter into a partnership WITH the client (not FOR) encouraging them to take charge of their own health. And it builds a healthy, therapeutic and trusting relationship. Thanks for this summary! So great:)
